Diagnostic Evaluations / Re-Evaluations
Costs for diagnostic evaluations vary depending on the nature and complexity of the client's needs.
Below are estimated total costs based on common areas of concern:
ADHD - $850
Learning disorders - $1,300
Learning disorders & ADHD - $ 1,500
Intellectual disability - $1,500
For diagnostic evaluations, you will receive a Good Faith Estimate detailing services and costs.
Insurance
Screenings, academic testing, IQ testing, and consultation services are considered academic services, and are not covered by insurance.
Some components of a diagnostic evaluation may be covered by insurance with authorization. We are currently in-network with Ohio's CareSource Medicaid and Ohio Medical Mutual. A doctor's referral is required prior to scheduling. Contact your carrier to verify your coverage. Ask if they cover evaluations for the area of concern (learning disorder, ADHD, etc.) They may ask for billing codes, and the most common billing codes we use for our diagnostic evaluations are: 90791, 96136, 96137, 96130, and 96131. Take note of any specific billing codes they may provide to you. We are happy to work with you to provide any documentation that may help.
For those with other insurance carriers who offer coverage of out-of-network services, a Superbill can be provided to you upon request. You can submit this documentation to your insurance provider to see if partial reimbursement is available.
Payment Options
For services under $500, payment is due at the start of the first appointment. Cash, check, credit or debit cards are accepted.
For diagnostic evaluations without insurance, 50% of the total service cost (detailed in the Good Faith Estimate) is due at the first appointment. The remaining balance is due at the feedback session.
A payment plan is also available upon request for diagnostic evaluations. A $200 deposit is required at the first appointment, with the remaining balance divided over 12 months of payment with no interest. A credit card authorization for automatic payments is required to be on file for payment plans.
