Diagnostic Evaluations / Re-Evaluations
The intake appointment is $200. Costs for diagnostic testing vary depending on the nature and complexity of the client's needs. You will receive a Good Faith Estimate detailing services and costs prior to your testing appointment. Below are estimated costs based on common areas of concern:
ADHD Evaluation (without academic assessment) - $650 - $900
Learning Disorder Evaluation - $1,200 - $ 1,700
Intellectual Developmental Disorder- $1,700
Insurance
Screenings, academic testing, Gifted/IQ testing, and consultation services are considered academic services, and are not covered by insurance.
Some components of a diagnostic evaluation may be covered by your insurance. We are currently in-network with CareSource Marketplace, and Ohio Medical Mutual, and Ohio Medicaid (fee-for-service only). A doctor's referral is required prior to scheduling, and prior authorization may be required. Contact your carrier to verify your coverage of psychological evaluations for the area of concern. They may ask for billing codes, and the most common billing codes we use for our diagnostic evaluations are: 90791, 96136, 96137, 96130, and 96131. Take note of any requirements that they may provide to you.
For those with other insurance carriers who offer coverage of out-of-network services, a Superbill can be provided to you upon request. Contact your carrier to determine benefits and prior authorization requirements.
Payment Options
For screenings and educational services, payment is due at the start of the first appointment. Cash, check, credit or debit cards are accepted. Screenings and educational services are not covered by insurance.
For diagnostic evaluations, any applicable co-pay is collected at the For those paying out of pocket, $200 is due at the intake appointment. The remaining balance is due at the feedback session. Cash, check, HSA, credit or debit cards are accepted.
A 6-month payment plan is also available upon request for diagnostic evaluations without insurance. A credit card authorization for automatic payments is required to be on file for payment plans.
